In short. Observation, skin care and sun protection, tightening classes, strength and posture work, then removal of skin. Only the last rung removes any skin.

Key points

  • Everything below this line changes the skin's quality or the shape beneath it.
  • Everything above it removes tissue.
  • That is the point at which the decision stops being about maintenance and becomes about an operation with a scar and a recovery.

There is a ladder of options for loose abdominal skin, and it is short at the top. Only one rung removes skin, and everything below it does something else.

Observation

The first rung is to have the skin assessed, record what was found, and do nothing. Retraction continues slowly for a year or more after weight settles, and it is more complete in younger skin and after a smaller loss. Waiting costs nothing, and it genuinely changes the answer in some people.

Skin care and sun protection

Moisturiser, barrier care in the fold, and protection from sun support the quality of the skin you have and reduce further damage. They tighten nothing. This is worth doing for its own sake rather than as a step towards a result, and it is also the preparation that makes any later plan safer.

Non-surgical tightening classes

Radiofrequency, ultrasound and microneedling produce a modest change in skin quality and surface texture, and some patients see a small change in looseness. None of them removes any skin. On an abdomen where the problem is a genuine excess of skin, this is the rung where money is most often spent on something that cannot deliver.

The rung where the ladder changes kind

Everything below this line changes the skin's quality or the shape beneath it. Everything above it removes tissue. That is the point at which the decision stops being about maintenance and becomes about an operation with a scar and a recovery.

Strength and posture work

Muscle sits under the skin, and building it changes how the abdomen reads without changing the skin at all. After a large weight loss many people have lost muscle as well as fat, and rebuilding it improves strength, posture and shape.

Removal of excess skin

This is the only rung that takes skin away, and it is usually performed together with a repair of the abdominal wall where the wall is also stretched. It leaves a long scar, it carries a recovery measured in weeks, and the result is judged months later rather than in the first weeks. Where laxity continues round the back and the flanks, the plan is staged over a longer period.

What the ladder does not contain

It does not contain a rung that removes fat and tightens skin at the same time, because removing fat from under loose skin leaves more skin relative to what is beneath it. It does not contain a cream or a device that removes skin. And it does not contain anything that restores the skin you had before it stretched.

Do not work through the ladder if there is

  • A rash in the fold that keeps returning, or skin that is raw or weeping.
  • A sore in the fold that will not heal, or skin breaking down.
  • A lump in the abdominal wall that does not reduce when lying flat.

What the devices can and cannot change is set out in What Firming Devices Do Not Do for an Abdomen. Why removing fat is not removing skin is explained in Why Fat Removal Does Not Tighten an Abdomen.

Frequently asked questions

What does the first rung actually consist of?

An assessment, a recorded measurement, and nothing further. It is the right answer where the laxity is mild, where a pregnancy is planned, where the weight has not settled, or where the appearance does not trouble you enough to justify a procedure. It leaves every other rung available, which is not true of anything already done.

Are the non-surgical tightening treatments worth having at all?

They produce a modest change in skin quality and surface texture, and they are worth having if that is what you want. They do not remove skin, and an abdomen with a real skin excess responds least of all to them. Where the complaint is a fold of excess skin, a course of them is money spent on something they cannot do.

Does anything on the ladder work for the whole abdomen?

Skin removal does, and it is usually planned together with a repair of the abdominal wall when the wall is also stretched. Where laxity continues round the back and the flanks, the plan is staged rather than performed in one operation, and the sequence is set with you rather than for you.

Risks & Limitations

  • Removing fat from under loose skin leaves more skin relative to what is beneath it, so the fold can look deeper.
  • No cream, device or fat-removal procedure removes skin. Only excision does.
  • Scars from skin removal are long and do not disappear. They mature over 6–12 months.
  • Further weight change and a further pregnancy can alter a result.

When to Seek Care

  • A rash in the fold that keeps returning, or skin that is raw or weeping.
  • A sore in the fold that will not heal, or skin that is breaking down.
  • A lump in the abdominal wall that does not reduce when you lie down.
  • Difficulty breathing or chest pain — seek emergency care immediately.
